Introduction

In compliance with the Data Protection Act 2018 I need to inform you about how I use any data collected from you. This includes what data I hold about you, why I need it, how I might use your data, whom I might share it with and when I will destroy it.
 
What information do I hold, why do I have it and who might I share it with?

Personal Details
At your initial treatment you will complete a consultation form which will provide me with your name, address, date of birth and various means of contacting you with regard to your treatment such as email address, mobile and home phone numbers.

From time to time I will send out exclusive offers to existing clients via a newsletter. You are given the choice of opting into receiving these newsletters at your initial consultation and may change this preference at any point by emailing alli@acmassage.co.uk. I will never send out unsolicited marketing material.

Health Related Data
Consultation Form – all clients/patients must complete a consultation form, so I have a full understanding of your medical and health history. This I need for insurance reasons, but it is to check that it is safe to treat you. It also helps me understand what medical issues you have and how best I can treat you or whether treatment is contra-indicated for any reason.

Treatment Records – these are my notes detailing what I have found in my assessments, what treatment I have given you and how you felt immediately after that treatment.
 
Retention
It is a condition of my Insurance Policy through FHT to retain client records for at least 7 years following the last occasion on which treatment was given. In the case of treatment to minors, records shall be kept for 7 years after they reach the age of 18.
I may need to share your data with authorised legal, regulatory and insurance authorities, if required to defend myself. This will be the professional membership body I am registered with and the insurance company I hold my professional indemnity insurance with.
After 7 years following the last occasion on which treatment was given, I will destroy all of your records by shredding them.

How and where do I hold your data?
All Consultation forms and Treatment records are on paper only and kept in a locked metal filing cabinet in my house. I use Shedul software to arrange appointments with you and the only information on this system is your name, mobile number and email address. Shedul have a strict privacy policy which you can view here https://www.shedul.com/privacy.html 

I currently do not enter any other information online about you or your treatment but should this change this will take place on a password controlled laptop / password protected online storage cloud. Only I will have access to these records and I will take all appropriate steps to protect the confidentiality, integrity, availability and authenticity of your data.

You have:
the right of access to your personal data;
the right to object to the processing of your personal data;
the right to restrict the processing of your personal data;
the right to rectification of your personal data;
the right to erasure of your personal data;
the right to data portability (to receive an electronic copy of your personal data);

My Rights
In exercising your Individual Rights, you should understand that in some situations I may be unable to fully meet your request, for example if you make a request for me to delete all your personal data, I may be required to retain some data for taxation, legal, regulatory and insurance purposes.
You should understand that when exercising your rights, a substantial public or vital interest may take precedence over any request you make. In addition, where these interests apply, I am required by law to grant access to this data for law enforcement, legal and/or health related matters.

Complaints
If you are dissatisfied with the way in which I process your personal data, you have the right to complain to the UK’s Data Protection Supervisory Authority, the Information Commissioner’s Office (ICO). We also use a tool called “Google Analytics” to collect information about your use of the Site. Google Analytics collects information such as how often users access the Site, which pages they visit, when they do so, etc. We use the information we obtain from Google Analytics only to improve our Site and services. Google Analytics collects the IP address assigned to you on the date you visit sites, rather than your name or other identifying information. We do not combine the information collected through the use of Google Analytics with personally identifiable information. Google’s ability to use and share information collected by Google Analytics about your visits to this Site is restricted by the Google Analytics Terms of Use and the Google Privacy Policy.

Third-party collection of information

Our policy only addresses the use and disclosure of information we collect from you. To the extent you disclose your information to other parties or sites throughout the internet, different rules may apply to their use or disclosure of the information you disclose to them. Accordingly, we encourage you to read the terms and conditions and privacy policy of each third party that you choose to disclose information to. 

This Privacy Policy does not apply to the practices of companies that we do not own or control, nor to individuals whom we do not employ or manage, including any of the third parties which we may disclose information to as set out in this Privacy Policy. 

Updates or amendments to this Privacy Policy

We reserve the right to periodically amend or revise the Privacy Policy; material changes will be effective immediately upon the display of the revised Privacy policy. The last revision will be reflected in the "Last modified" section. Your continued use of the Platform, following the notification of such amendments on our website, constitutes your acknowledgment and consent of such amendments to the Privacy Policy and your agreement to be bound by the terms of such amendments.
